XML : How do I fix this layout?

I have been stuck on this for around 4 hours now, and am really frustrated. I want to simply have a layout like this:

And I have tried this in several different ways in my XML, but it never works. Whenever I try, the buttons and textviews move out of place, like this:

Please help me figure out how to get my layout like the first image. Here is the code that I tried:
